Caring Advocates maintains our own storage and reference system for general information, where requests can be made for immediate FAX or Internet downloads of all Advance Care Planning Documents.

For both, you must enter the patients "Unique Identifying Number" that is engraved on the medallion.

The Natural Dying DNR Medallion is optional in many states.

Several states on this page is to give you an opportunity to consider the variety of wording specified by various statutes.
Ohio and Maryland give their residents a choice of two wordings, and thus we MUST offer two kinds of medallions for each state. But in many cases, what statutes require can be considered a minimum and if you see something you like, you can request we create a custom medallion for you.

Caring Advocates' medallions are highly reflective and either gold or nickel plated. They are difficult to photograph but easy to read, as demonstrated by this Ohio medallion.
