Caring Advocates, in addition to maintaining our own storage and reference system for general information, maintains a partnership with the U.S. Living Will Registry where requests can be made for immediate FAX or Internet downloads of all Advance Care Planning Documents.
For both, you must enter the patients "Unique Identifying Number" that is engraved on the medallion.

One reason we display medallions from several states on this page is to give you an opportunity to consider the variety of wording specified by various statutes.
Ohio and Maryland give their residents a choice of two wordings, and thus we MUST offer two kinds of medallions for each state. But in many cases, what statutes require can be considered a minimum and if you see something you like, you can request we create a custom medallion for you.
